Comment: removed the chapter table. should we remove things that are "done" from the list? i would argue leaving things that we will not do so we have at least have a record of things that we considered but didn't do.

This keeps track of who has completed their proofread of each chapter. (Our SVN repository will keep us from simultaneous edits.)
